Real-world performance and utility of a noninvasive gene expression assay to evaluate melanoma risk in pigmented lesions.
This study evaluated the clinical performance and utility of a noninvasive gene expression assay, the pigmented lesion assay (PLA), in routine dermatology practice for assessing melanoma risk in pigmented skin lesions. The PLA detects expression of two genes, LINC00518 and PRAME, associated with melanoma, using adhesive patch sampling, aiming to reduce unnecessary biopsies. Retrospective analysis of 381 patients (51 PLA-positive and 330 PLA-negative) across four US dermatology sites showed that 100% of PLA-positive lesions underwent biopsy, identifying 19 melanomas (all early stage, in situ or stage 1), yielding a number needed to biopsy (NNB) of 2.7 and a biopsy ratio of 1.7 benign lesions per melanoma. Meanwhile, 99% of PLA-negative lesions were managed by surveillance, with no melanomas detected in follow-up biopsies. The PLA demonstrated estimated sensitivity of 95%, specificity of 91%, and a negative predictive value (NPV) exceeding 99%, outperforming the traditional visual assessment and histopathology (VAH) pathway which has lower sensitivity (~84%) and specificity (<30%). Dual positivity for LINC and PRAME correlated strongly (93%) with melanoma diagnosis, while single-gene positives were less predictive. The study highlights PLA’s ability to guide clinicians in biopsy decisions, significantly reducing unnecessary biopsies and excisions while maintaining high melanoma detection rates. Limitations include the assumption that unbiopsied PLA-negative lesions are true negatives and a lower melanoma prevalence in the cohort, but ongoing extended follow-up studies aim to address these. Overall, PLA offers a noninvasive, objective, and accurate tool that transforms melanoma risk assessment from subjective and invasive toward precise and patient-friendly clinical management.
